The Yadkin County Health Department Vital Records Section registers all deaths and births occurring in Yadkin County, as well as the issuance of Burial Transit Permits and Disinterment/Reinterment Permits.

The funeral director, or persons acting as such, is responsible for filling a Certificate Of Death with the health department where the death occurred within 5 days after death.  The hospital or medical facillity where the birth occurred is responsible for filling a Certificate Of Live Birth (for each live birth) with the health department within 5 days after the birth.  Home births need to file for a birth certificate directly through the health department.

When the birth or death certificate is received, the Deputy Registar checks the certificates for errors; missing or inconsistent information makes any necessary corrections and then files a copy of the certificate with the Yadkin County Register Of Deeds and teh original certificate with the State Vital Records Office.
