The Maternity Care Coordination Program is available during your pregnancy and the two-month period after the delivery of your baby.

The Maternity Care Coordination Program provides the following services: 

• Help with coordination of checkups *
• Help with coordination of transportation *
• Childbirth and parenting classes *
• Information on hospital care for your baby's delivery *
• Help with coordination of health care for you and your baby after the birth (including birth control) *
• Referral to other programs like WIC *
• Information on prescription drugs and dental care *
• Breast feeding counseling *

Eligibility:

If you are eligible for Medicaid then you are qualify for Maternity Care Coordination.
