Health: Child Service Coordination |

This voluntary program helps families find and use community services that are designed to support families. There is no charge for participation. If you have children from birth to age five who qualify for the program, you child service coordinator will work with you in meeting your family's needs. These can help with finding medical care, transportation, child care and/or financial aid.
Goals:
- To get the parent in touch with services for children and families.
To support the children in reaching their developmental potential.
To help ensure that children are raised in healthy, safe, nurturing homes.
All children birth to 5 years of age with special needs are eligible. Other children in this age range may be eligible if there are significant concerns about their growth or development.
How can the Child Service Coordination program support you and your child?
Your child service coordinator has information about a wide variety of family oriented resources. He or She will:
- Discuss family strengths and concerns through home visits, telephone calls and other personal contacts.
- Identify programs, services and resources that meet your family needs.
- Provide developmental screenings.
- Serve as a link between you and your child's doctor or nurse.
- Suggest ways to strengthen parent-child relationships.
- Introduce you to parent support programs when available.
- Offer you encouragement and support.
