Care Coordination for Children
Care Coordination for Children (CC4C)
Care Coordination for Children is a program offered at no charge for certain children from birth to 5 years of age who:
  • Have chronic medical conditions
  • Are in stressful situations long term
  • Have been referred by the child’s doctor

The goal of CC4C care management is to improve the health of your child by linking you to effective, efficient services that will meet the needs of your child and family. For eligible families who want CC4C services, a CC4C care plan is designed by the family and the CC4C care manager.

CC4C services are provided or assured by local health departments in partnership with Community Care of North Carolina and North Carolina Division of Medical Assistance.

CC4C Managers
CC4C care managers work with families through:
  • Home visits
  • Phone calls
  • Visits at the doctor’s office
  • Other types of contacts

How often these contacts occur depends on the needs and desires of the family. What the CC4C care manager can do:
  • Ask you about your child and family needs, concerns, goals, and questions
  • Work with you to be sure you have a medical home (doctor) for your child and / or support you in building a strong link with them
  • Work with every health care provider that your child sees to be sure he/she gets the best treatment.
  • Help you get answers to questions
  • Recognize you as the expert on your child
  • Offer referrals to other community agencies to address your family’s needs, concerns, and goals
  • Offer information on health and family topics, including child growth and development
  • Support you in being your child’s best advocate
