How to Make an Appointment

To make a WIC appointment, please call 336-849-7910. Please be prepared to provide the name, date of birth, address, and telephone number for all persons needing an appointment.

When to Make an Appointment

Appointments should be made by calling 336-849-7910 or coming into your local WIC office between 8 a.m. and 5 p.m., Monday through Friday.

What You Will Need to Bring to Your Appointment

At every certification and subsequent certification appointment you will need to provide ALL of the following:

  • Physical presence: This includes yourself (if you are a pregnant or post-partum Woman) or the potential WIC applicant.
  • Current Identification (examples of what you will need to bring, only one is needed for each person applying):
    • Birth certificate
    • Immunization record
    • Medicaid card
    • Mothers Verification of Facts
    • Social Security Card
    • eWIC card
    • A valid driver's license
    • Current work/school or military identification
  • Proof of Residence (where you live) examples of what you will need to bring (only one is needed for each person applying, a P.O. box will not qualify):
    • A utility bill, no older than 30 days (i.e. gas, water, electric, etc.)
    • Rental or mortgage agreement
    • Medicaid card
    • A valid driver's license
  • Proof of income (before taxes) for all household members (examples of what you will need to bring):
    • Medicaid card or
    • Proof of Work First eligibility or
    • Letter of Food Stamp certification
  • If none of the above applies, bring:
    • Pay stub, no older than 30 days
    • Tax return for those who are self employed
    • Proof of ALL money coming into the home (i.e. copy of child support check, SSI check, etc.)
  • For nutrition or follow-up and mid-certification assessment appointments:
    • Proof of identification (see examples above)

Who Should Come to the WIC Appointment

The type of appointment will determine who is required to be present. When you schedule your appointment, ask the WIC staff member assisting you to verify who should be present at the next visit.

Caretakers & Proxies

Caretakers

Caretakers must be at least 18 years of age. He/She should be someone that you can trust, and should be familiar enough with the WIC participant to answer basic questions about the participant's eating behaviors and habits.

Proxies

You will be given an opportunity to designate a proxy at each certification appointment. If you choose to do so, he/she will be able to pick up the food benefits. When a proxy comes in your place, he/she will need to bring the following:

  • ID for themselves
  • eWIC card

Prescriptions

A prescription is required for the issuance of many products allowed through the WIC program. Specific medical documentation must be included on any prescription submitted to the WIC Program for consideration. All prescriptions are subject to WIC approval and provision based on program policies and procedures.

Situations that might require a prescription include:

  • An infant who requires formula other than contract milk-or soy-based infant formulas 
  • A child or woman who requires an exempt formula or medical food.
