Area Plan Survey

This survey is being taken to help develop a 4 year work plan for Yadkin county as well as 11 other counties for the development of local services and new programs needed for the adults 60+, individual with disabilities and caregivers. It would be appreciated for people to complete this survey so Yadkin county citizens needs can be expressed so that when the plan is developed, Yadkin county is represented.
